چکیده

Timely identification of patients with patent infarct-related artery (IRA) may permit rapid triaging to avoid unnecessary repeat thrombolysis or rescue percutaneous coronary intervention (PCI). Various biomarkers have been proposed to assess IRA patency status. Our group found that the ratio of serum myoglobin levels obtainned before and 60-min after initiation of thrombolytic therapy may provide an early indication of the IRA patency status. A 60-min myoglobin ratio of >or= 4.0 indicated the probability of a patent IRA of 90%, suggesting that follow-up diagnostic modalities such as emergency coronary angiography to determine the IRA status may be unnecessary when these dynamics of myoglobin are observed.

Analysis of creatine kinase, CK-MB, myoglobin, and troponin T time-activity curves for early assessment of coronary artery reperfusion after intravenous thrombolysis.

BACKGROUND Thrombolysis has become the standard therapeutic approach in patients with acute myocardial infarction. To identify patients who may benefit from early invasive procedures, reliable noninvasive assessment of success or failure of thrombolytic therapy is mandatory. METHODS AND RESULTS In a prospective study in 63 consecutive patients undergoing thrombolysis for their first myocardia...

Transient release of lipid peroxidation products as a non-invasive marker of successful reperfusion after thrombolysis for myocardial infarction.

OBJECTIVES To evaluate an increase in plasma concentration of thiobarbituric acid reactive substances as a non-invasive biochemical test of reperfusion after thrombolysis and to investigate the relation between the inflammatory response after acute myocardial infarction and the production of the substances. METHODS Venous samples were taken from 19 patients receiving thrombolysis for acute my...

Myoglobin, Creatine-Kinase-MB and Cardiac Troponin-I 60-Minute Ratios Predict Infarct-related Artery Patency After Thrombolysis for Acute Myocardial Infarction

RESULTS Angiography at 60 min showed a patent IRA (TIMI flow grade 2, 3) in 344 and occluded IRA (TIMI flow grade 0, 1) in 98 patients. The median serum T60 concentration, the ratio of the T60 and T0 serum concentration (60-min ratio) and the slope of increase over 60 min for each serum marker were significantly higher in patients with patent arteries compared with patients with occluded arteri...

Serum myoglobin for the early non-invasive detection of coronary reperfusion in patients with acute myocardial infarction.

The ideal non-invasive method for detecting coronary reperfusion has not yet been established. In 63 patients with acute myocardial infarction, serum myoglobin and creatine kinase-MB were measured every 15 min. Thrombolytic treatment was given (n = 52) and acute coronary angiography showed a patent infarct-related artery in 49 patients while 14 patients had no coronary reperfusion. Median time ...

Kinetics of myoglobin release and prediction of myocardial myoglobin depletion after coronary artery reperfusion.

To better define the usefulness of blood myoglobin measurements in evaluating the effectiveness of attempted thrombolysis, we studied the kinetics of myoglobin entry into and removal from the circulation after coronary artery reperfusion and the relation between directly measured depletion of myocardial myoglobin after coronary occlusion and reperfusion and the amount of depletion predicted fro...
